Oral History Release Form

Subject to the terms, conditions, and restrictions set forth in this agreement, I, ____________________, do hereby give, donate, and convey to the Archives a nonexclusive, paid-up license throughout the world in the information and responses on tape, and in printed transcription of the tape (hereafter referred to as “the donated materials”) provided during the interview conduction by _________________________ on _________________________ at _______________________________. This donation covers all present and future copyright interests in the materials, including the right to reproduce such materials in copies of audiocassette, printed from, video, phono-records, and/or on the Internet, to distribute copies (in the aforementioned forms) to the public, to perform or display the work publicly, to prepare derivative works thereof, and to have others do so for research purposes.
